We welcome you to register and experience the benefits of being a patient with the Harrow Road Health Centre.

To register, please fill in the Patient Registration form or your medical card and hand it to one of our Reception Team along with a completed new patient questionnaire. Please note, until October 2014, the Practice is only able to register patients who live in its catchment area which is shown below. After this date, in accordance with national changes, the practice will be able to register patients irrespective of where they live.

You can complete the Patient registration form online if you prefer – it will save some time when you attend your health check. Pleaseclick here to complete the online form. Please note that you will still need to briefly attend the practice to complete your registration. You will need to provide documentation to confirm your current address (e.g. a bank statement or utility form), sign your registration form, complete a short health check form and book an appointment with one of our Nurses for a health check. Your registration will then be complete.

When you register, you will be booked to see the Practice Nurse for a routine health check. This enables us to record important information about your health and provide you information about protecting your health.

New patients will be given full information about the services we offer.

Choice Pilot

As the practice is participating in a national Department of Health pilot that allows people from outside the usual practice boundaries to register as permanent patients, we can now register patients no matter where they live.

For patients who live outside the practice boundary (the area highlighted in red in the map below), you can register with us on the understanding that:

• HRHC will not provide any home visits or any other form of care outside of the practice area

• The responsibility for urgent care / home visits when you are outside of our practice boundary will go to the PCT covering the area that you live in.

After registration with us, your local PCT will write to you to provide you with information about:

• How best to access local emergency care services • Who to contact if you need a home visit during practice “normal” opening hours • Arrangements for accessing “out of hours” services
